Faculty Intent to Participate Form

PLEASE NOTE --- the cut-off date for submitting this form is September 9, 2011. No forms will be accepted after that date.

Thank you for volunteering to participate in the Illinois State University Institutional Artifact Portfolio process for Assessing the General Education Program. Please note the new criteria.

    • No cover sheet will be required.

    • Single artifact submission for each class section taught - please pick the single assignment that best reflects the traits of the Life-Long Learning rubric.

    • This form must be submitted for each section of a course for which you will submit artifacts.

Upon receipt of this form, a University Assessment Services Staff Member will e-mail you to confirm the pick up date, time, and place for your artifacts of student work. Please note that the artifacts will be copied and the originals will be returned to your department's/school's central office within one business day.
